How do I send a referral to Zeal?

Fax the completed referral form to 877-329-9325, or send the prescription electronically to ZEAL (NPI 1093424905). Medication-specific forms are on our Referral Forms page, or call 888-412-ZEAL, option 2, for verbal orders.

How fast will my patient start therapy?

Speed is the point. We begin the benefits investigation and prior authorization as soon as the referral arrives, and we keep you and your patient updated at every step.

Do you handle prior authorizations and appeals?

Yes. Our team manages the prior authorization, documentation, and appeals so your staff does not have to.

What therapies and conditions do you support?

We focus on rare and orphan disease and specialty medications, with 23+ orphan drug launches supported, including ultra-orphan pediatric therapies. If you do not see a form for a specific drug, call us; we likely handle it.

How will I know what is happening with my patient?
