1. Print the Addyi referral form.

2. Give the form to your Addyi prescriber.
3. Have your prescriber send it to us: fax the completed form to 877-329-9325, or send the prescription electronically to ZEAL (NPI 1093424905).

That is it. Our team will reach out once a valid prescription is received to confirm your information and coordinate your shipment.

This product requires a valid prescription for shipment. Zeal Specialty Pharmacy cannot accept prescriptions faxed or emailed by patients.
